1) Resident semi-private and private furnished/unfurnished rooms
2) Basic Meal Service (three nutritious meals per day, physician prescribed diets and a minimum of
two snacks)
3) Laundry service
4) Cleaning service
5) Weekly linen changes (more frequent if necessary)
6) Transportation for facility-based activities or programs (resident requested transportation outside of
the facility-based activities or program will be charged as an optional service) Planned facility- and
community-based recreational activities
7) Assistance with activities of daily living
8) Observation, supervision and care of resident
9) Bedside care for a maximum of five days for minor illnesses (colds or flu)
10) Assistance with self-administered medication
11) Hygiene items of general use
12) Monitoring and appropriate reporting of residents needs and conditions
13) Arrangement of and transportation to medical and dental appointments
14) Special and Modified Diets as required per physician
15) Telephone service- will provide special communications equipment as required by residents
16) Adaptive devices for meal service
OPTIONAL SERVICES

We will provide the following optional services at the resident's request, the cost will be charged at the
posted rate given with the admissions agreement with a notice that rates are subject to change (30 day
notice will be given) for optional services based on increase in product or service cost.
